Complete Traveler’s Health Surveillance Form

Tanzania issued a Travel Advisory on October 17, 2026, mandating that all travelers to Tanzania must complete an Online Traveler’s Health Surveillance Form within the 24 hours prior to arrival in Tanzania.

Once complete:

Ensure you have saved a copy of the QR code or confirmation number to show at the airport.

If you have not completed this step prior to arrival in Tanzania, you will need to complete the form at the airport, which may result in delays.
